SNAP Tool for Mental Health - SNAP-MH Tool

The Support Needs Approach for Patients (SNAP) is a healthcare intervention which seeks to enable person-centred care for patients with a range of conditions. SNAP uses an evidence-based tool (the SNAP Tool), to help patients identify and express their support needs which are then discussed with their health or social care professional. Below is some brief information about SNAP and the SNAP Tools – for the full guidance on SNAP please visit: https://thesnap.org.uk/ .
Technology No. i987

The SNAP Tool

The SNAP Tool is a brief, yet comprehensive, evidence-based tool which comprises a set of questions relating to areas in which patients may require more support. There are two versions of the SNAP Tool: one for patients with predominantly chronic or progressive conditions (15 questions: the original SNAP Tool), one for those with predominantly mental health conditions (17 questions: the SNAP-MH Tool) and one for those with hearing loss (15 questions: the SNAP-HL Tool)..

This webpage relates to the SNAP-MH Tool (Mental Health; 17 questions).

To obtain a licence to use the original SNAP Tool for patients with predominantly chronic or progressive conditions (15-questions) please go to SNAP Tool.

To obtain a licence to use the SNAP-HL (Hearing Loss - 15 questions) please go to SNAP-HL

The SNAP-MH Tool is completed by the patient to help them consider and express their support needs. The completed tool is then used as the basis of a needs-led conversation with their health or social care professional (within the 5-stage SNAP intervention) to try to address those needs.

An inspection copy of the 17-question SNAP-MH Tool is shown below, however a licence is required for SNAP-MH Tool use. Once a licence is requested you will be able to access a usable version of the tool.

The SNAP-MH Tool can be used in clinical practice, in research (to identify support needs), in systematic reviews (as frameworks of support needs), or in teaching (e.g., on support needs or person-centred care). To use the SNAP-MH Tool in clinical practice (even if within a research study) or teaching you are required to complete the freely available SNAP training (see below).

The SNAP-MH Tool is free for use within not-for-profit organisations e.g., the NHS.

The SNAP-MH Tool is currently only available in English. A licence is required to translate the tool.

Why do we need SNAP?

Some patients find it difficult to tell health or social care professionals about their support needs, either because of limited time during the consultation or because they are unsure what is appropriate or relevant to mention.

The SNAP-MH Tool acts as a prompt for patients, and the 5-stage SNAP intervention (which the tool underpins) gives patients “permission” to raise and discuss their support needs with health or social care professionals and gives them a means to do so. It makes person-centred care visible to the patient: the patient can see that there is a process for identifying and addressing their support needs.

Who can use the SNAP-MH Tool?

Health or social care professionals who have completed SNAP training (see below) can use the SNAP-MH Tool within the 5-stage SNAP intervention to enable person-centred care. The SNAP-MH Tool can also be used as a standalone tool in research studies to identify broad areas of unmet support need in patients with relevant conditions. The tool can also be used as a framework for systematic reviews relating to support needs, and in teaching (e.g., on support needs or person-centred care).

Licensing

In order to secure a licence to use the SNAP-MH Tool in clinical practice or teaching, you must first complete (as a minimum) (1) the 90-minute, free to access, online SNAP training (Unit 1), where you will be shown how to use the tool within the SNAP intervention and (2) read the brief 'SNAP Training Supplement for Mental Health Clinicians'; both can be found on the SNAP website training page. There are additional training resources on the page that you may find useful.

SNAP training is not required to use the SNAP-MH Tool in research provided that the tool is not being used to deliver clinical care in any way (if clinicians will be delivering the 5-stage SNAP intervention within the research study then SNAP training is essential).

Once you have completed any required training please log back in here and complete the licence fields - you will then be able to download the relevant SNAP Tool and any associated documents. All the downloads will be accessible through the 'Supporting documents' link at the foot of this page. There are five types of licence:

  1. clinical
  2. research (provided that the tool is not being used to deliver clinical care within the study - if it is then a clinical use licence will also be required)
  3. translation
  4. teaching
  5. systematic review
